Prednisolone is the active metabolite of prednisone.^[1]

Prednisolone is a corticosteroid drug with predominantly glucocorticoid and low mineralocorticoid activity, making it useful for the treatment of a wide range of inflammatory and auto-immune conditions such as asthma, uveitis, Pyoderma Gangrenosum, rheumatoid arthritis, ulcerative colitis, temporal arteritis and Crohn's disease, Bell's palsy, multiple sclerosis, cluster headaches, vasculitis, acute lymphoblastic leukemia and autoimmune hepatitis, systemic lupus erythematosus and dermatomyositis.

It can also be used as an immunosuppressive drug for organ transplants and in cases of adrenal insufficiency (Addison's). Prednisolone acetate ophthalmic suspension is an adrenocortical steroid product, prepared as a sterile ophthalmic suspension and used to reduce swelling, redness, itching, and allergic reactions affecting the eye.

Corticosteroids inhibit the inflammatory response to a variety of inciting agents and probably delay or slow healing. They inhibit the edema, fibrin deposition, capillary dilation, leukocyte migration, capillary proliferation, fibroblast proliferation, deposition of collagen, and scar formation with inflammation.

Hydrocortisone, is a corticosteroid hormone or glucocorticoid produced by the zona fasciculata of the adrenal cortex, which is one of the two parts of the adrenal gland.It is released in response to stress, or to a low level of blood glucocorticoids. It is controlled by hypothalamic secretion of corticotropin-releasing hormone (CRH), which in turn triggers pituitary secretion of Adrenal Corticotrophic Hormone (ACTH); ACTH is carried by the blood to the adrenal cortex where it triggers glucocorticoid secretion. Its primary functions are to increase blood sugar and stores of sugar in the liver as glycogen, aid in fat, protein and carbohydrate metabolism, and to suppress the immune system (and regulate lymphocytes). Various synthetic forms of cortisol are used to treat a variety of different illnesses

Dexamethasone is a potent synthetic member of the glucocorticoid class of steroid drugs. It acts as an anti-inflammatory and immunosuppressant. Its potency is about 20-30 times that of the naturally occurring hormone hydrocortisone and 4-5 times of prednisone.

Anti-inflammatory:Dexamethasone is used to treat many inflammatory and autoimmune conditions, such as rheumatoid arthritis.

It is also given in small amounts (usually 5-6 tablets) before and/or after some forms of dental surgery, such as the extraction of the wisdom teeth, an operation which often leaves the patient with puffy, swollen cheeks.

It is injected into the heel when treating plantar fasciitis, sometimes in conjunction with triamcinolone acetonide.

It is useful to counteract allergic anaphylactic shock, if given in high doses. It is present in certain eye drops – particularly post-eye surgery drops – and as a nasal spray (trade name Dexacort), and certain ear drops Dexamethasone is used in transvenous screw-in cardiac pacing leads to minimize the inflammatory response of the myocardium. The steroid is released into the myocardium as soon as the screw is extended and can play a significant role in minimizing the acute pacing threshold due to the reduction of inflammatory response. The typical quantity present in a lead tip is less than 1.0 mg.

Dexamethasone is often administered before antibiotics in cases of bacterial meningitis. It then acts to reduce the inflammatory response of the body to the bacteria killed by the antibiotics (bacterial death releases pro-inflammatory mediators that can cause a response which is harmful to the patient), thus improving prognosis and outcome.^[2]
